Ureteral metastasis from prostate cancer is an exceedingly rare manifestation of disease progression and may mimic a primary tumor of the upper urinary tract. The limited number of reported cases underscores both the uniqueness of this localization and the substantial diagnostic challenges it presents. This report describes a clinical case of delayed metastatic ureteral lesion in a prostate cancer patient 13 years after radical prostatectomy. At primary diagnosis, the neoplasm in the ureter was regarded as urothelial carcinoma. Following surgical resection and immunohistochemical analysis, the tumor was confirmed to be a metastasis from prostate adenocarcinoma (PSA+, CK7–/CK20–). This case illustrates the diagnostic complexity of atypical prostate cancer progression and underscores the need for a comprehensive pathomorphological evaluation to confirm the diagnosis and guide subsequent treatment decisions.
