Original research Occupational Burnout Syndrome in Emergency Healthcare Workers

There were evaluated the features of occupational burnout syndrome formation in healthcare workers. Questionnaire materials according to V. V. Boyko method of emergency healthcare workers were used in the research. It was shown that the occupa­tional burnout syndrome in physicians was predominantly determined by emotional-moral disorientation, while the symptoms characterizing reduction of emotional interpersonal relationships prevailed among doctor's assistants.

Original research Age-Associated Changes of T-Subpopulation Content in Patients with Diabetes 2 Type

The purpose of the study is to determine the markers of immunosenescence in the diabetes 2 type. It was investigated the blood of 184 patients (age 50–90 years) by the method of polychrome flowing cytometry and enzyme immunoassay. By the method of cluster analysis the subpopulation of CD4+, CD8+ and the activated CD CD3+HLA–DR+ T-cells (Tact) as the most informative indicators were selected. The groups of patients with uniform changes of this complex were formed. The groups included patients with the different stages of activation or deficit of CD4+ and/or CD8+. Tact values, which exceeded the norm, were identified with varying frequency in all age groups, including those with normal absolute number of CD4+ and CD8+. In this case detection rate was 100% (activation) and decreased to 33 % in immunodeficiency of CD4+ and CD8+. The proportion of individuals with activated immunity decreased with aging from 41 % (50–59 years) to 22% (80–90 years) and with immunodeficiency — increased from 43 to 75%, respectively. The chosen complex of indicators reflected the age-associated reaction of the T-cells in individuals to the chronic inflammation in diabetes type 2 and can be used as a diagnostic marker of the immunosenescence stage.

Original research Balanced Anesthesia Based on Isoflurane Inhalation in Reconstructive Operations on the Carotid Arteries

The aim of the research was to examine the influence of isoflurane inhalation on the circulatory dynamics, encephalic bloodstream and oxygen balance of the patients' brain during the carotid endarterectomy. 26 patients were examined. There were detected the parameters of central and systemic circulatory dynamics, the speed of the bloodstream in the middle encephalic artery in M1, sector, the results of transcranial cerebral oxymetry. The first stages of the operation, the stage of temporary breakoff of the blood circulation in damaged artery territory were accompanied with moderate lowering of indicators of circulatory dynamics. In the process of reinitiation of the bloodstream the reperfusion of brain declared itself moderately. Isoflurane anesthesia to 1 MAK (minimum alveolar concentration) in combination with phentanyl allows to maintain appropriate pain management, stable circulatory dynamics and well-timed awakening of patients during carotid endarterectomy.

Method Forecasting of Outcomes of Distal Radius Meta-Epiphysis Fractures by Means of Instability Criteria Estimation

he purpose of the research is to create the effective system of diagnosis and treatment of patients with distal radius metaepiphysis fractures to achieve as quick and complete recovery of functions of a damaged upper limb as possible. The research is of clinical and radiological character and is based on the study of the influence of 7 known and 2 newly suggested instability criteria of the fracture on clinical and X-ray parameters of the wrist with improperly fused distal radius meta-epiphysis fractures of 129 patients. There has been developed a diagnostic algorithm that takes into account the instability criteria (with varying degrees of influence on clinical and X-ray parameters of the wrist joint) and allows to determine the optimal treatment of patients of this profile.

Review Biodegradable Materials Application in Traumatology and Orthopedics (Review)

It was performed the analysis of published data on the use of products made of biodegradable materials in traumatology and orthopedics. Currently more and more implants made from polylactic and polyglycolic acids are widely used, in particular, in trauma — pins and screws. Low frequency of the osteolysis and foreign body reactions to the use of these devices was demonstrated, the duration of the controlled period of their decay was up to 18 months. The advantage of these implants is the ability to complete resorption, which eliminates the need for surgery to remove the device and tissue damage accordingly. There were presented the data of clinical studies which contributed to the implementation in practice of biodegradable orthopedic products. It was concluded that further research on the use of products made of biodegradable materials in traumatology and orthopedics is needed.

Original research Medical and Social Problems in Reproductive Loss Prevention of Population in the Current Conditions (by the Example of Omsk Region)

Reproductive losses in the Omsk region were significantly higher than in the whole of Russia, and in 2010 amounted to 7.1 per 100 live births (in the Russian Federation — 5.0). In the sociological study conducted to examine the effective­ness of the provision of social support was attended by 640 women with children under three years of age. There were statistically significant differences in the physical development of the newborn (weight and body length), and the level of material well-being in the experimental and control groups. It was made a conclusion that not only medical but also social indications should be taken into account when providing targeted social assistance to pregnant women.

Original research Nutrition as a Factor of Reproductive Impairments Prevention and Healthy Offspring Reproduction

In the report the authors present data provided evidence that a qualitative inadequacy of pregnant women nutrition con­tribute to the development of iron-deficiency anemia, pregnancy and delivery complications. Anemia leads to a fetopla­cental failure, development of anemia in newborns, metabolic processes disturbances which result in various pathological conditions. Higher prevalence of anemia in rural pregnant women and platypodia in children living in families with a low level of living wage proves the role of social factors in ensuring a healthy generation. The authors demonstrate the need to examine protein-vitamin-mineral status of population in the Family Planning Center, health centers and antenatal clin­ics. It is proved the effectiveness of nutrient deficiency correction in pregnant women with the use of natural concentrated products made of vegetable and protein-vegetable raw materials.

Original research Protection and Improvement of Reproductive Health of Workers with High Risk Professions

Protection and improvement of reproductive health of the population of Russia depends on the complex approach to these issues. An interdisciplinary approach to the problem of health care in reproductive health can improve diagnosis, the development of preventive and therapeutic measures, blurring the lines between basic and applied research. The problems of the scientific substantiation of a system of risk management of high risk professions workers' reproductive health as a basis for the health of the posterity and proposed preventive measures for the purpose of improving the demographic climate in the country are considered in the article.
